Any adult patient in Shropshire, Telford and Wrekin and North Powys who has an incurable illness and requires palliative care or end of life care can be referred to Severn Hospice for support.

GPs, hospital consultants and any other healthcare professional can make a referral to any of our services. All referrals will be assessed and prioritised by clinical need and the ability of the hospice to meet those needs.

Severn Hospice is not an emergency service but we can accept out of hours emergency admissions if the patient’s needs dictate and we have the capacity to admit the patient. In the case of an out of hours or emergency admission please call.
